Common Reasons for a Hospice or Palliative Care Referral
Has a serious illness
Emphasis on quality of life
Pattern of frequent hospitalizations or trips to the emergency room
Shows a decline in ability to perform daily activities on their own or needs more care than can be easily managed
Requires specialized pain management or symptom control
Has had an increase in falls, has progressive weight loss, or shown decline in cognitive status
Has infections and other signs of decreased health
