Our Care Team is dedicated to providing personalized care, ensuring the comfort and dignity of our patients and their families

  • Physician & Nurse Practitioner

    Collaborates with the patient's primary physician to coordinate care and oversee all aspects of treatment provided by the care team

  • Hospice Nurse

    Focuses on managing the individualized care plan of each patient, emphasizing comfort and the alleviation of pain and other distressing symptoms

  • Social Worker

    Provides personalized support to patients and families through empathy, advocacy, and problem solving; helps navigate community resources and assist with advanced care planning

  • Hospice Aide

    Assists with personal care needs such as bathing, dressing, and grooming to enhance patient comfort and dignity

  • Spiritual Care Counselor

    Offers nondenominational spiritual support and counseling to patients and their families, addressing emotional and existential concerns

  • Bereavement Counselor

    Helps patients and their families navigate the emotional journey of grief and loss, offering coping strategies and support

  • After-Hours Nurse

    Ensures continuity of care during nights and weekends, providing essential support and interventions as needed

  • Volunteers

    Engages in a wide range of activities including companionship, emotional support, and respite care
