Life is unpredictable. Serious illness and accidents can happen unexpectedly. If you find yourself unable to communicate, who will speak for you?
Completing an Advance Directive can offer peace of mind for both you and your loved ones. An Advance Directive is a legal document that outlines your personal and medical healthcare preferences and designates an advocate to speak on your behalf when you are unable to do so. It's essential for everyone over the age of 18 to have an Advance Directive in place.
By proactively considering and communicating your treatment preferences ahead of time, you can alleviate the burden on your loved ones of having to guess your wishes. Most importantly, you retain the autonomy to make deeply personal healthcare decisions for yourself.

POLST is a portable medical order that outlines your preferred care in an emergency. It is effective immediately upon signing and follows you across all healthcare settings. Easily understood by all healthcare professionals, including first responders, POLST is typically presented on a bright green form and must be signed by a physician, APRN, or PA, along with the patient or their representative.
